Have you tried turning on the channel summing? If there is no low range signal going to channel 2 input (like the rear speaker line) there is nothing the LC7 can do about that. Unless you sum the channels.
If you get something when you turn on channel sum, then you know it is the rear speaker...
That is exactly what I used for my setup. Speedwire to go from head unit to amp and 14 gauge speaker wire from amp to speakers. 10 gauge speaker wire for the sub.

If you use the GTO for your AC amp, you can use that to trigger your mono amp. Run a wire from the Remote In/Out terminal on the AC amp to the remote terminal on the mono. Should work fine, at least it has in my other cars.

The sub should be pretty easy. You don't need to tap anything. Your Audio Control amp should have a sub output that you can run RCAs from to your sub amp. As long as you have a full range signal to your AC amp (kick panel feed), you can feed the sub amp through the AC amp.
