You are correct, it is a head restraint and not a head rest. It is set at some angle (I don't remember what it is anymore) so that in a rear impact collision, the chances of whiplash are minimized. In all honesty, that is why they are uncomfortable most of the time.
I call shenanigans, when I wad rear ended the headrest did fine. It was the pitch forward followed by the face plant into the steering wheel that sent me to the hospital. My forehead hit the top of the steering wheel my nose the horn button. And yes, my seat belt was firmly in place but loosened when I was pitched back.
