Form actually got sued by alpharex in the last few years as the projectors are a patented item to alpharex the original company that came out with those lights in the first place. Form either stole the design or had the manufacturing facility sell them a light without the alpharex logo in the light from what my sponsor over at alpharex told me a while back.A few things I can say about my 2 years experience with Form lighting...
The Good:
I do love the look of the lights when they are working. And they are bright! Much brighter than the factory signatures.
I have never had an issue with the low beams or the high beams.
Love the start-up sequence and being able to switch from white or orange DRL's.
They have one of the best warranty and support departments of any company I have ever dealt with. When I did have an issue, they asked for a quick video and a picture of the serial number. After that a replacement was mailed out immediately. No questions, no hassle.
The bad:
I bought them in Feb 2024 and 2 weeks after installation I started to get quite a build up of condensation in both headlights. (replaced under warranty)
The new replacements lasted about 10 months and the halos started burning out. I cant remember which side went first but both headlights were replaced under warranty.
The same happened again (halos flickering and burn out) to both headlights. These lasted about 7 months. Again new lights sent out under warranty and I have them sitting here in the box. Since they seem to have the most issues in the winter, so I will wait until spring 2026 to install them.
So having 3 sets of these over the course of 2 years I would not say they have been very reliable for me. Even though the warranty is excellent, I would rather have reliability and not as bright. I have read Alpha Rex, Morimoto, Oracle and others all having similar issues, so I will be installing my OEM signatures or maybe looking for a set of OEM braptors when this next set goes bad.
I'm skeptical if Diode dynamics even makes these bronco Form lights. They are suspiciously similar to Archaic and another lower brand I cant recall. They look like the same housing, projectors, and the start-up sequence is an exact match. Diode dynamics isnt known for having reliability issues, but my Form lights have had plenty.

I have installed my OEM's because this set of Form's did not make it through this winter. There's still a couple months of winter left and the turn signal on the passenger side was almost totally out. I had no signal light at all. Dangerous.
I decided to rip them apart before tossing them and I didn't see any diode dynamics labels or logo inside. They are pretty cheaply made inside IMO. There are two main boards. The first one looks like it talks to the bronco and runs the headlights. The second looks like it runs the halos and the start-up sequence. The halos have shaped boards behind them with dual color led's. None of these boards have any conformal coating or PCB glue. They are exposed components so any moisture that is in there is going to get on the electronics. The headlights all have fans behind each projector so it’s no wonder I kept getting condensation in them. There are two large packets of desiccant stuck on the inside of the housing so I guess they were trying to prevent moisture but it didnt work for the climate here. I'm not sure if these release moisture in the heat or once they absorb all they can they just stop working. Either way my experiance hasn't been good with them. I just dont believe Diode dynamics actually makes these. I think they are rebranded.

This is why I have my OEMs installed in my Bronco.This is why I have my OEMs sitting in a box just in case although I don't have any issues with mine. I am at roughly 15 months with them.
I absolutely agree with you on their support. They are fantastic to deal with. A couple pictures and a new one is sent out right away.My experience has been great. I have had an issue or two and they've replaced lights upon receipt of the issue.
Nothing is bombproof and all aftermarket headlights are mass produced, so support matters more to me than mfg quality when the light quality is great. Recently had an issue with the halo on a light, no questions asked new one in route.

I firmly place the blame with @Ford Motor Company on this one. These premium vehicles come with top of the line electronics, borderline ridiculous power, fantastic drivetrains and they give us ...reflector based lights?!?!?6)...it's the supplier of the headlights who should ultimately be at fault but that's in the past now...
