Had mine installed by dealer. They gave me the card and also logged the code in there system under my VIN. I would go to the dealership and see if they can help you.

I agree, if the door was sprung open past the stop, the hinge or mounts could be bent and this will cause the door to rub the fender ( I have seen it happen before) a good body shop can adjust the door and replace the bent hinge, if needed.

Well I ordered the keypad online at ford accessories with instalation. Had the pickup dealer local "Freehold Ford" they completed the install mounting and programming at the ordered price. Great dealership. (BTW I did not purchase my truck from them)
