Just the one so far, but it was seemingly unavoidable. It held up really well too, considering the circumstances.

Sorry about that. The bolts definitley worked themselves loose and caused this to happen. I am having threadlocker put on these bolts as security. I have spoken to other people that also suggested thread locker.
